How we help
Our team of clinical professionals provides virtual care when:
- You're recently discharged from the hospital and need follow-up care
- You need problem-focused, timely care while looking for a kidney doctor (nephrologist) in your area
- You have CKD stage 3-5 and need support managing your condition

Strategic interim care management for you and your patients
Interwell Medical Group offers specialized interventions for CKD stage 3-5 patients who are unable to access timely care. Our virtual services are designed to provide crucial support during transitional periods, ensuring patients receive necessary care while they wait for access to local nephrology care. By addressing urgent clinical needs and optimizing care for patients, we aim to improve health outcomes and reduce hospital readmissions.
Patients we serve
- Who are without a nephrologist
- Who have recently been discharged from the hospital and are unable to see their provider in a timely manner
- Who were triaged by the Interwell Health Care Team
How we help
- Providing focused virtual interventions for CKD patients through comprehensive care visits that include nephrology care (for those patients without a nephrologist), medication optimization, post-hospital discharge follow-up, and lab ordering
- Transferring care to a local nephrologist as quickly as possible, ensuring the continuity of care for patients
- Ensuring follow-up care is provided in close collaboration between our clinical professionals and local providers, to ensure the best possible care outcomes
